Our Adult Community Support service provides practical, non-regulated support to help adults stay active, connected and involved in everyday life.
This support is designed for adults who may need encouragement, reassurance or practical assistance to access the community, take part in activities, build confidence outside the home and stay socially connected.

Adult Community Support may be suitable for adults who need practical support to access the community, attend activities or stay connected with others.
This may include adults who feel isolated, lack confidence going out alone, need support with community routines, or would benefit from structured support to take part in agreed activities.

Support may include:
Attending community activities
Going for walks or outings
Visiting local groups, clubs or social spaces
Support to attend agreed appointments or activities
Building confidence outside the home
Encouragement with routines and independence
Helping adults stay socially connected

We start by understanding the person’s needs, preferences, routines and goals. We then agree what support is required, where it will take place and how often support is needed.
Support is provided in a respectful and person-centred way, with attention to dignity, safeguarding, communication and individual choice.
